Preventing Avoidable Readmissions Together (PART) Program

Patients being discharged from the hospital with a clear understanding of their after-hospital care instructions, including how to take their medicines and when to make follow-up appointments, are 30 percent less likely to be readmitted or visit the emergency department than patients who lack this information, according to an AHRQ-funded study.
PART offered proven educational resources within an inclusive community where diverse providers from all corners of the state could brainstorm solutions, trade stories, share successes and frustrations, and give each other advice and support. It was about building a map to smoother transitions of care – together.

​In the two years since founding PART, South Carolina member hospitals collectively reduced targeted readmissions by 15%, avoided 1,400 preventable hospital stays, and ranked 9th in the country in the improvement of readmissions
